   Find out who handles Privacy Act requests, and file one requesting a copy of your OPF.  Or, if you already have a copy of your file, write that person and send them copies of the last SF 50 and the letter, and explain the discrepancies and request that the record be corrected in accordance with the provisions of the Privacy Act.  If memory serves, they've got 10 days to fix it (although that might have been extended in recent years).  The task will undoubtedly be assigned to HR, but it shouldn't take them long to make things right.  Good luck with it!
